Non Surgical Homoeopathic Treatment for Avascular Necrosis of Femoral Head

My colleague, Homoeopathic Doctor, 42Yrs, Male, called me on 5^th August 12, having severe pain in Right Hip Joint since 2 months. Actually, he took some Anti-inflammatory medicines without any effect. Being a Doctor & worked with the Orthopedician, and observing a recent case of Osteo-Sarcoma in his OPD, he was worried about the Cancer. He himself has done at first X-Rays of both hips AP & LATERAL views which looked absolutely normal. He took Ars Alb, Thuja, Rhus Tox as suggested by some senior homoeopaths without any result. As pain was increasing affecting his daily work, not able to squat or sit down & limping due to severe pain, he has done MRI of both HIPS which was showing:

·        Altered marrow signal in the right femoral head & the neck is most likely suggestive of transient bone marrow oedema or early changes of Avascular necrosis of femoral head.

·        Moderate right hip joint effusion.

His chief complaint was

·        Right Hip Joint Pain: Heaviness as well as throbbing

< by cold draft

< by squatting

< at the start of motion

> continued motion

·        Right Hand, Dorsally a flat wart since 6 months.

Past history:

·        H/O Bilateral Renal Calculi

·        Left heel Corn operated in childhood

Family History:

·        Diabetes: Father & mother

·        Herpes Zoster: mother

State of Disposition & Mind:

Being a Homoeopath, he has explained his mental state very nicely in a descriptive style. He started explaining his past disappointment in love before marriage around in 1993-94 which made a lot of impact on him. He got married with a Doctor in 1996. He has 2 daughters. He has explained regarding his past life to his wife. As a result of depression of the disappointment in love, he joined a Religious commune & got much attached with it thereafter till today. Because of his attachment with the religious commune, he was spending much time in the temple. This has created some disputes between him & his wife. She was expecting his responsibility regarding the daughters as well as his attention at home which she was missing. There used to be some fights or quarrels between them. Whenever there used to be a dispute, he used to escape from home to temple. He told that he is much lazy & not proper in following the timings. Because of the disappointment & dispute, he became indifferent towards everything. He used to be much conscientious regarding every little matters as well as relations with parents as well as with patients. During the interview, he asked me number of questions. He used to frown a lot while talking.

Before the MRI was done, he had a fear of Cancer in his mind & when the diagnosis Avascular Necrosis was found, he started worrying about the future. Due to pain, he was saying – ‘when these pains will stop, I am fade-up from these pains.’ While talking, he told number of his thoughts, theories regarding his life.

He was seen by the Orthopedician who advised him to take Anti-inflammatory along with complete rest till the pain settles, avoid sitting down, squatting etc.

Physical Examination: 

Weight: 66.5 Kg

P: 78/Min

B.P.: 127/79 MM Hg

Face: Greasy, has a big religious mark on his forehead, brown spots on his face.

Clinical Examination:

·        Not able to lift the Right Leg easily as it was much painful

·        Not able to bend his right leg & thigh on abdomen.

·        Not able to squat in lying down position

·        Tenderness +++ in Right Hip Anteriorly

This is my personal 20 years experience that Bio-chemic Salts when used on the basis of their pathogenetic actions, works very fast & hasten the cure process. Bio-combination No.26 by name is for Easy Parturition but it contains following tissue salts which cover the patient’s pathology perfectly.

The question always arises that why not Sepia only?

 This is definitely a debatable issue. My answer to the question is that Sepia alone can also be sufficient but it will take its own time to work even though you used it in any potency. The group of the patient generally I use to see is not that much capable of understanding the difference between the Palliation & the Cure. They want the difference in the pain & an early result. This was not the issue for this case but as a routine I prescribed it to him also. And why not we should be result oriented? Since 20 years I am working on these Bio-chemic Salts & those being the semi dynamic, found to produce very fast results at the cellular level. If the marrow oedema settles immediately, patient becomes very comfortable to continue the treatment for long duration. Calc Fluor is the medicine which has the capacity to dissolve the osteophytes. I have experienced it number of times regarding this action. In cases of Osteo-arthritis, Spondylosis, Spinal Canal Steniosis one must think of this remedy.

28/08/2012:

·        Pain in right hip: 80% better;  occasionally < start of motion > continued motion

·        Limping while walking still persist

·        Able to squat 70% on lying down

·        Able to bend the hip

·        Tenderness on Right Hip: Absent

·        Wart on right hand decreased in size

24/09/2012:

·        Pain in Right Hip: Absent

·        Able to squat in sitting as well as lying down posture

·        Limp absent

·        He danced in religious commune without any trouble

·        Able to bend his Hip

·        One hour played a Tabala in squatting position

·        Very much happy as he was able to perform all his duties & enjoy the hobbies

DISCUSSION

As this case was very Acute & Recent in appearance (Even though the disease is chronic in nature), proper Homoeopathic Management can bring such patients very fast in a gentle way.  

Now a day, I am treating Avascular Necrosis cases often & what I observed is Lachesis, Sepia & Calc Fluor appeared commonly in those cases. So, I studied the cases in retrospective manner to find out that whether I am becoming prejudiced in prescribing? But it was not the case. So, I studied the action of all three remedies & found that all of them have an action on Vascular System, Circulation, Venous System. I think, we must study the actions of our medicines even at cellular & tissue level so that we can make our Materia Medica more rich & more perfect.

Hope for Actinomycosis in Homoeopathy

Mr. AM, a 70 year old politician came to me on 30 July 1911 for the treatment of actinomycosis on left foot for 20 years. It was between joint of his great toe and 2nd & 3rd toe. There were recurrent swelling, effusion, and eruptions. It was operated earlier followed by long continued use of antibiotics and antifungals, which were used to affect his Liver Metabolism as well as increased Serum Bilirubin levels. He has to do his Liver Profile every 3 months. He also had fungal infection in the groin for which he used to apply local Antifungal along with steroid ointments. He was known Hypertensive since 15 years. He was having L5-S1 disc prolapse since 2002 for which he has consumed Allopathic Medicines. Since 10 years, he is having Sun Dermatitis. He has to wear full shirts for the same. He used to suffer a lot from Flatulence generally at 4 pm after eating farinaceous food. He is also suffering from Nasal Polyps which used to obstruct his nose at night.

Past History:
Angioplasy was done 5 years back.
Prostatectomy was done 4 years back.
Both Cataract were operated.  

Family History:
Father Hypertensive died 5 years back.

Personal history:
A typical Politician who was past minister sat in a chair with typical Pompous Attitude, Dominating, Dictatorial, Boasting about his achievements in a Egoistic tone. He was quiet Reserved used to frown a lot during conversation. He used to tell various Theories while explaining his disease. He is quiet Religious & used to follow religious aspects for longer period. He was very much Sensitive to all matters in the society as well as in the environment. He was quiet Doubtful about recovery of his disease.

Analysis of the Case:
In this case, thematic presentation also denotes the Lycopodium. After Repertorisation, the remedy was more confirmed. I have considered the miasmatic view i.e. Syphilitic aspect of the case & from Bio-chemic Medicines, I have considered the Calc Fluor. So, I have started the case as:

No recurrence of the swelling, eruptions, nodules.
Nasal obstruction as it is.

  Even though, there is no recurrence, his Ego was asking me where is the result? He was doubtful about the recovery & the way he asked me was having the tone of Domination & Egotism. So, I have increased the potency as:

Calc Fluor 6x od 2 Months

 6 December 1911:
No recurrence of the eruption or swelling or nodules.
His itching in the groin is increased & used to burn a lot < Night.
P: 80/Min B.P.: 144/76 mm Hg Wt: 88 Kg
Advised: Weight Reduction
Calc Fluor 6x od continued till the next follow-up as there was no recurrence which used to recur again & again earlier.

28 July 2012:
There is no recurrence of the eruptions, swelling, and nodules.
P: 88/Min B.P.: 150/86 mm Hg Wt: 89 Kg
But, he was still in doubt whether he will be cured completely or not. His dominating attitude which was much better in between again came up. His voice tone was again showing high pitch. He was not following the diet which was advised him to reduce the weight. So, I have again increased the potency of Lycopodium.

Discussion: After starting the Homoeopathic Medicines, I have stopped all the allopathic medicines for Actinomycosis. Only, Anti-Hypertensive was continued as he was not ready to reduce or stop it. Even after one year, there is no recurrence of the disease is observed. But still, the case cannot be considered as cured. But, it definitely shows the rays of hope in Actinomycosis with Homoeopathy.

A CASE OF LUMBO-SACRAL PROLAPSED INTERVERTEBRAL DISC WITH PARAPLEGIA...

Mr D.D.S., 47yrs, a technician of a Political Party, came to me on 12 Dec 12, walking with crutches supported by a partner, diagnosed as a case of Paraplegia with Feet Drop due to Vit B 12 deficiency along with diffuse posterior annular disc bulge at L3-4 to L5-S1 level.
He was not able to lift his legs, feet, toes.
He was not able to stand without the support.
He was getting numbness in thighs at night with sudden shock like sensation.

A CASE OF DUCHENNE MUSCULAR DYSTROPHY

A very affectionate child, 4.5years, a case of Duchenne Muscular Dystrophy, came to me on 16 Feb 2010, with tremendous weakness in muscles of upper & lower limbs with hypertrophy.
Due to weakness in the limbs, child wants to be carried after walking for little distance, very sensitive to pain. He is not able to climb up & downstairs otherwise he requires support.
His CPK enzyme levels were 33616 IU/L on 16/09/09. (Normal levels are 25-192 IU/L).

He was on Prednisolone 40mg/day on tapering dose since 6 months to 10mg/day without any improvement in the weakness. In fact his muscle weakness is increased.
He is having flat foots.

Nature wise, very affectionate; restless; shy; likes to play in company; does not like to go to school; when questioned, he becomes dull, unable to answer either due to timidity or difficulty on thinking; getting irritable on very small matters; fears dark; with active memory; found to be disobedient, biting his nails & putting the fingers in mouth, continuously moving here & there in the consulting room.
His mother told that he is homesick, moody child but desires to travel a lot.
He eats pencil a lot. He sleeps on abdomen. He still wets his bed at night.

Duchenne muscular dystrophy
• Duchenne muscular dystrophy (DMD) is a recessive X-linked form of muscular dystrophy, which results in muscle degeneration, difficulty in walking & breathing, and death.
• The incidence is around 1 in 3,600 boys.
• Females and males are affected, though females are rarely affected and are more often carriers.
• The disorder is caused by a mutation in the dystrophin gene, located in humans on the X chromosome (Xp21).
• The dystrophin gene codes for the protein dystrophin, an important structural component within muscle tissue.
• Dystrophin provides structural stability to the dystroglycan complex (DGC), located on the cell membrane.
• Symptoms usually appear in male children before age 5 and may be visible in early infancy. Progressive proximal muscle weakness of the legs and pelvis associated with a loss of muscle mass is observed first. Eventually this weakness spreads to the arms, neck, and other areas.
• Early signs may include pseudohypertrophy (enlargement of calf and deltoid muscles), low endurance, and difficulties in standing unaided or inability to ascend staircases.
• As the condition progresses, muscle tissue experiences wasting and is eventually replaced by fat and fibrotic tissue (fibrosis).
• By age 10, braces may be required to aid in walking but most patients are wheelchair dependent by age 12.
• Later symptoms may include abnormal bone development that lead to skeletal deformities, including curvature of the spine. Due to progressive deterioration of muscle, loss of movement occurs, eventually leading to paralysis.
• The average life expectancy for patients afflicted with DMD is around 25, but this varies from individual to individual.
Pathogenesis of DMD
• Dystrophin is responsible for connecting the cytoskeleton of each muscle fiber to the underlying basal lamina (extracellular matrix) through a protein complex containing many subunits.
• The absence of dystrophin permits excess calcium to penetrate the sarcolemma (cell membrane). Alterations in these signaling pathways cause water to enter into the mitochondria which then burst.
• Muscle fibers undergo necrosis and are ultimately replaced with adipose and connective tissue.

Prednisolone stopped by the patient’s parents a week back as there was no relief.

Follow Up (26/02/10)
He suffered from Mumps on right side on 20th Feb & recovered itself within 4 days without any medicine.
Playing with friends, weakness decreased a lot after the mumps.
Remedy continued in bd doses for 15 days.

I already advised them that he might suffer from any infectious disease after starting the treatment based on understanding from Predictive Homoeopathy. So, if anything happens, please, don’t give any medicine. (Ref: Verbatim by Dr Praful Vijaykar, Chapter 14)

• Why the Mumps did came up? Is it a good or bad sign?
• Mumps is a tumor virus like HSV, EBV & CMV.
• Why are they known as tumor virus in the allopathic world? Because when they infect & go to the cell, they cause a change in mitotic cycle in such a manner that extra growth occurs in a cell. The cell starts growing excessively giving rise to tumors. For us it is a Sycotic virus.
• Therefore, sycotic virus being thrown out of the body is a good sign in this case. The tumor viruses when thrown out to clear the internal malady or dysfunction are exteriorization....
• ...So, just rejoice & lay your hands off if tumor virus appears. The virus is in the neuro-ectoderm & is thrown off. This is a excellent sign. These viruses, especially tumor virus harboring in the nerves or muscles are thrown off through the prochordal plate. So the patient will first get some throat problem & then on the body.

Weakness in limbs much decreased.
He started running as well as climbing staircase without support.
He was getting thick nasal discharge since week with occasional dry cough.
His confidence level increased.
Remedy continued in od dose for 1 month.

He is walking without asking for carrying. He is playing cricket, running here & there, climbing up & down stairs without any support. Now he is found as very happy looking child.
Remedy continued in od dose till 19/01/11 when his CPK enzyme levels were measured & found to be 1075 IU/L.

Follow Up (19/01/11)
His weakness in the limbs was absent.
His bed wetting also stopped.
He complained of Pain in his knees while squatting.
Otherwise, he is playing, running, climbing up, & downstairs without any support.
Remedy Prescribed: Calc Phos 30 3 doses followed by Placebo daily.

Recent Follow Up (20/10/11)
Weakness in the limbs absent.
No bed wetting.
Pain in his knees while squatting was absent.
He is playing, running, climbing up & downstairs without any support enjoying his life.

Existence is the state or fact of existing,

He feels hopeless when he realizes that he is in a situation from which he can not be saved. He has been wrong suffered but due to Helplessness he can not do anything. This is MISERABLE EXISTENCE of Carcinocin patient.

MIND - DELUSIONS - cancer, has a: Carc
If a patient’s health is deteriorating day by day and it is not getting diagnosed, he starts believing that he has cancer meaning, this dangerous disease will take away his life. This state is because of the extreme suffering and the feeling that he has some serious incurable disease.

Cancer = Death like suffering rather sufferings of death in living body
And carc patient has fear about this disease.

MIND - FEAR - cancer; of: Carc
People believe that this disease can not be cured by any means. It spoils or deteriorates your body. It spreads to other organs and slowly spoils the whole body. Cancer is a state of helplessness where the person does not die but keeps sufferings for a long time till death finally takes over him. It is very painful condition where many would rather prefer dying than live such a miserable life.
He has fears of such INCURABLE situations or diseases.
MIND - FEAR - death, of - cancer; of: Carc
MIND - FEAR - disease, of impending - incurable, of being: Carc
Once he would get it he can not be cured. This fear makes him restless.
MIND - FEAR - recover, he will not: Carc
This is one side of explanation. On other side, if he loses faith of parents of any authority he fears he can not RECOVER this faith.
Recover means to get or find back, to regain a former condition after a loss, to make up for.

He always remains in the thoughts of disease, the miserable situation of life etc. In this situation one can not console him. He never gets pacified with assurance that everything will be alright.

MIND – INCONSOLABLE: Carc
Once he recognizes that he is in grievous state, it is unbearable to him. He thinks or attempts suicide due to this sufferings. He feels life is boring now.
It’s better to die. He wants death by him or others.
“Muze nahi jina, mar dalo muze”

MIND – ENNUI: Carc
Ennui is the feeling of being bored by something tedious.
Tedious means so lacking in interest as to cause mental weariness.
MIND - WEARY OF LIFE: Carc
Weary means exhausted or get tired through overuse or great strain or stress, lose interest or become bored with something or somebody, physically and mentally fatigued.

Development from the expectation of others

In this world of competition every parents expect their child should earn money, fame, etc. They expect that their child should do much more in life (when they see the achievements of children around them esp. of relatives, neighbors). They have “Extraordinary expectation” from child. Carcinosinum person evolves from this situation as a child.

MIND - AILMENTS FROM - expectations of parents too high: Carc

Here, parents do not care whether their child is having ability to achieve, compete, etc. Out of selfish motive to gain esteem in society, they expect child should complete their wishes and please them AT ANY COST!!!
At this time, what is the passion of child, what they really want, what is the aim of them, or what they want to become is ignored. By keeping aside all the wishes (of children), it is important that they should fulfill their parents wishes.

MIND - DESIRES - suppressing his desires: Carc

He does not get chance to recognize his abilities, what can he do.

In this situation, he is dominated by parents. He is abused. His upbringing is done very harshly. They behave to him very rudely. They reproach, reprimand, and punish him. He remains sad due to this “hard influence of others.”

Due to this torturing, he becomes angry but he swallows his anger.

MIND - ANGER - abused; after being: Carc

Humiliation means:
1) State of disgrace or loss of self-respect
2) An instance in which you are caused to lose your prestige or self-respect

Asthmatic respiration means smothering feeling; inability to breath as per one’s wish i.e. can not live according to one’s wish- ‘Ghutansi Hoti Hain.’

WHY? Because he is helpless on emotional and mental level:

MIND - HELPLESSNESS; feeling of - mental level; on: Carc

He feels if he revolts against them, he will not be accepted, cared i.e. he will be rejected. Already he has suffered a lot where he has not got emotional attachment for which he starved since long time.

Reject means refuse to accept or acknowledge. His proposals, his emotions, his passions, are always rejected.

Actually, a Carcinocin child feels:
MIND - CARESSED; being - wants to be caressed: Carc

But what happens? Only sadness comes to him and he feels unfortunate.
MIND - SADNESS - love; from disappointed: Carc
MIND - UNFORTUNATE, feels: Carc

He goes into the grief state, but this grief is not expressed.
MIND - AILMENTS FROM - grief - silent grief: Carc

Though he is sad, he can not weep (weep mean to make visible, to express) and this grief remain for long time.
MIND - WEEPING - cannot weep, though sad: Carc
MIND - GRIEF – prolonged: Carc

Then it leads to ‘hollowing out’ of him.
MIND - GRIEF - undermining the constitution: Carc
Undermine means:
1. Damage or weaken esp. gradually or insidiously
2. Dig or excavate beneath so as to make it collapse.

‘Forsaken feeling’ is the theme of Carcinocin


He feels that:
‘I am not loved by anyone.’
‘No one pay attention to me.’
‘No one praise for what is done by me.’
‘I have no friend.’
‘No one understands me.’
‘He is discarded by world.’
Now, he fears for aloneness that seems to come. He feels very sad when he feels alone.

MIND - DELUSIONS - alone, being - belong to anyone; she did not: Carc

Abandoned means forsaken by owner; leave someone who needs or counts on you.

When he is alone, he is in need of company, caressing, kind words, etc.

In this state of sadness, he feels like to talk to someone, wants to share. Once he tells or shares for a long time (because it is aggregated for a long time and to the deepest level) he feels better.

MIND - TALKING - amel the complaints – prolonged: Carc

Out of it he desires sympathy and affection from others. He longs for sympathy and affection which is not given to him.
MIND - SYMPATHY, compassion - desire for: Carc
MIND - AFFECTION - children craving for: Carc

He expects someone should talk ‘sweet words’ with him, should listen to him, which gives him relief.
MIND - CARESSED; being - wants to be caressed: Carc

Meticulousness of Carcinosinum

The meticulousness of Carcinocin patient is MALIGNANT (i.e. it is too much extreme, hyper).
It is said that Over Perfectionist person prone to CANCER. So is the disposition of Carcinocin patient.

The rubrics in this category are:
MIND – FASTIDIOUS: Carc2
MIND - FASTIDIOUS - disease; in: Carc
Disease is the condition in which you are unable to do normal activities i.e. dis-ease; discomfort feeling. Means even in any condition (worst, discomfort) he has to be fastidious. It is out of helplessness feeling.

MIND - FASTIDIOUS - work; in his: Carc
MIND - CLEANNESS - mania for: Carc
Mania- irresistible motive for a belief or action i.e. it is a passion.
Cleanness- without dirt or other impurities Here dirt means disgraceful gossip about the private lives of other people. ‘Jo bhi karega itna perfect karega ki koi ungli na utha sake.’

MIND - ORDER - desire for: Carc
Order is the quality or meticulous nature.

MIND - PROPER - too - children; in: Carc4
Proper means marked by suitability or rightness or appropriateness.
Too means to a degree exceeding normal or proper limits.

MIND – TASTEFUL: Carc
Tasteful means showing good aesthetic judgment or appropriate behavior; free from what is unbecoming. Synonym neat which means clear and organized.

MIND – TIDY: Carc
Tidy means
1. Marked by order and cleanliness in appearance or habits e.g. a tidy person, a tidy house.
2. Put (things or places) in order e.g. Tidy up your room!
3. Neat and clean.

WHY he is so?
The fastidiousness of Carcinocin patient is not inborn. It is conditional.
MIND - UNTIDY - perform; except when he has to: Carc

He is fastidious for the wishes of others. He is tidy till he has to impress others, please others. Otherwise he is not tidy enough. If he does not behave like this, he will confront PROBLEM!
This is reality of Fastidiousness of carc. He has TO BE fastidious to please others esp. parents for the fear of rejection, reproaches etc.
MIND - PLEASING - desire to please others: Carc2
MIND - PLEASING - desire to please others – parents: Carc
MIND - PLEASING - desire to please others - parents – father: Carc
Father is the highest authority, dominating person in the family. Otherwise he will be reproached, criticized which Carcinocin does not want.
This is the ONLY reason why Carcinocin becomes fastidious.

MIND - FEAR - reproaches; of: Carc
‘Koi chillaega, datega ye teri vajah se hua hain’ is bat ka dar rahata hain.

MIND - FEAR - dirt; of: Carc
Dirt means disgraceful gossip about the private lives of other people.

MIND - SENSITIVE - criticism; to: Carc3
‘Log meri taraf kichad uchalenge, jo main nahi chahata.’


To please others, Carcinocin works hard, does not take rest until everything is done meticulously.
MIND - REST - cannot rest when things are not in the proper place: Carc

Due to such hard work prostration comes upon him.
MIND - PROSTRATION of mind - working too hard: Carc
MIND – TORPOR: Carc

Then he feels to take rest.
MIND - REST - desire for: Carc

But, fear and anxieties remains in his mind that if he does not work or perform properly, he will be in problem. So, even in disease condition he is fastidious.
If he unable to fulfill the expectation of others esp. parents he will be troubled, hurt, reproached, reprimanded, punished, and lastly will be rejected. And he will not able to gain position in the eyes of parents.
MIND - FEAR - recover, he will not: Carc

MIND - RESPONSIBILITY - taking responsibility too seriously - children; in: Carc
MIND - CONSCIENTIOUS about trifles - children; in: Carc

While doing so, he suppresses his desires, anger, and emotions which are the big SADNESSES in his life.
But, when it becomes unbearable for him, he revolts against AUTHORITY. Now, he does not listen to anyone. He becomes indifferent to the feelings of others.

Sharp means clearly defined, ending in a sharp point, harsh.

Touch means to come in contact with; a communicative interaction; affect emotionally.

Previously, he desires Caressing, but now due to dissatisfaction, he is reluctant to sympathy and consolation.

He is in discontentment now.

This feeling takes him away from humankind and he develops affinity toward nature.

He becomes artistic due to which he feels happy because here he can live life according to HIS WISH. There is nothing expected from him.

The material organism, without the vital force, is capable of no sensation, no function, no self-preservation1, it derives all sensation and performs all the functions of life solely by means of the immaterial being (the vital principle) which animates the material organism in health and in disease.

1 It is dead, and only subject to the power of the external physical world; it decays, and is again resolved into its chemical constituents.

Dr Prasad Rasal:

In this aphorism, Dr Samuel Hahnemann tries to explain the details regarding the Vital Force & the Vital Principle. He explains that the material organism, without the vital force, is capable of no sensation, no function, no self-preservation. The material body derives all sensation and performs all the functions of life solely by means of the immaterial being (the vital principle) which animates the material organism in health and in disease.
So, the animation to the immaterial being is because of the Vital Principle i.e. nothing but the Soul. If, it is lost, the human being is called as a dead. What remains after the death is nothing but the material body which decays & again becomes part & parcel of this universe.
